Payment is expected at the time that services are rendered. We ask that you bring your Insurance and ID card with you on each visit in order to expedite billing.
For obstetrics, hospitalizations, and surgeries, we are happy to file insurance claims and disability forms for you.
We are participating providers with most PPO plans, Primecare Medical HMO, Medi-cal and Medi-cal HMO plans. Please call our office for more information.
In addition, you may print out new patient forms to fill out in advance and bring to your first office visit.
